Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans to improve their quality of life.

Responsibilities
  • Conduct thorough health assessments, including medical history, diagnosis, and treatment, in patients' homes
  • Develop and implement personalized care plans to address identified health risks
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, with prescription authority
  • Board certification in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills

Travel Requirements

This role requires frequent travel to patients' homes, with a valid state driver's license and adequate automobile insurance coverage.
