We are seeking a dedicated and compassionate Social Worker to join our team at Regency of Fremont. As a Social Worker, you will play a vital role in providing medically-related social services that help our guests attain and/or maintain their physical, mental, and psychosocial well-being.
Key Responsibilities:
- Conduct thorough social services evaluations and assessments to identify the needs of our guests.
- Develop and implement individualized care plans to address the social, emotional, and psychological needs of our guests.
- Provide counseling services and support to help our guests cope with life changes and challenges.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support for our guests.
- Maintain accurate and timely documentation of social services provided.
Requirements:
- Current social work licensure in the state.
- Bachelor's degree in social work required; master's degree preferred.
- One year of supervised social work experience in a healthcare setting is preferred.
