POSITION SUMMARY:

The Social Work Case Manager plays a crucial role in collaborating with the healthcare team to ensure effective management of patient care. This position focuses on minimizing delays and addressing potential denials in patient services.

KEY RESPONSIBILITIES:

  • Actively coordinate with community organizations to tackle issues such as abuse, neglect, substance dependency, mental health challenges, and adoption processes.
  • Facilitate connections to financial assistance and resources for patients in need.
  • Engage with multidisciplinary teams to enhance the overall patient experience and outcomes.

QUALIFICATIONS:

  • A Master’s degree in Social Work is mandatory.
  • Experience in Social Work ranging from 2 to 4 years is essential.
  • Prior experience in Medical Social Work is highly desirable.
  • Certification as a Licensed Clinical Social Worker (LCSW-C) is required.
