About the Role

We are seeking a highly skilled and compassionate Intensive In-Home Qualified Professional to join our dynamic team at Southeastern Integrated Care LLC. As a key member of our Intensive In-Home Team, you will play a vital role in providing high-quality, individualized services to children and adolescents with serious emotional, behavioral, and/or substance use disorders.

Key Responsibilities
  • Provide Intensive In-Home Services, a team approach designed to address the identified needs of children and adolescents.
  • Assist individuals and families in developing critical daily living and coping skills.
  • Collaborate with the clinical team to develop and implement Person-Centered Plans (PCPs) that promote recovery, symptom reduction, and increased coping skills.
  • Document daily progress notes and maintain accurate records.
  • Participate in clinical supervision and training to ensure ongoing professional growth and development.
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a Human Services field or a related field with 2 years of experience in the MH/DD/SAS population, or a Bachelor's degree in a field other than Human Services with 4 years of experience in the MH/DD/SAS population, or a Registered Nurse (RN) with 4 years of experience in the MH/DD/SAS population.
  • Valid driver's license and personal vehicle insurance coverage.
  • Ability to work in various environments, including residential, school, workplace, and community settings.
