Job Summary

The RN Case Manager role at The Waters is a critical position that supports the Director of Health and Wellbeing in the day-to-day clinical operations of the community. This role is responsible for planning, organizing, and coordinating the nursing process to provide exceptional care to residents based on The Waters' commitment to service, presence, and partnership.

Key Responsibilities
  • Clinical Leadership: Manage resident care with specific knowledge and experience in assessments, symptom management, crisis intervention, service recovery, and family collaboration.
  • Collaboration and Communication: Collaborate with other healthcare entities, such as providers or therapeutic disciplines, in the assessment of resident care needs; adjust the resident service plan to correlate with best-recommended care through an interdisciplinary mindset.
  • Clinical Education and Development: Identify clinical education needs that are outside of routine requirements, such as new resident-specific skill competencies.
  • Comprehensive Assessments: Conduct comprehensive clinical assessments to identify person-centered and holistic approaches to care needs and promote successful outcomes.
  • Team Management: Assign and oversee appropriate delegation of services to subordinate team members.
  • Quality and Integrity: Maintain appropriate clinical data integrity through best-practice documentation standards and appropriate use of data-based systems.
  • Emergency Response: Respond immediately to unexpected/emergency situations that arise outside of the daily assignment, including but not limited to: emergency response system calls from residents, security issues, and falls.
Requirements
  • Education: Graduate of an accredited school of nursing required.
  • Licensure: Licensed as a Registered Nurse (RN) in good standing by the state in which the community is located.
  • Experience: Preferred two years of general nursing experience within a geriatric setting.
  • Certification: Dementia certification or willingness to become certified.
  • Leadership Experience: Preferred previous supervisory and leadership experience.
