We are seeking a highly skilled Home Infusion Nurse to join our team at Naven Health. As a Home Infusion Nurse, you will play a critical role in delivering exceptional care to patients in the comfort of their own homes.
Key Responsibilities- Plan, provide, and document professional nursing care for patients in accordance with physician orders and established policies and procedures.
- Assess patient needs and physical status each skilled visit through health data assessment and patient interview.
- Provide effective and safe teaching using patient-centered care approach, modifying teaching modalities to fit patient/family learning style.
- Practice effective and timely communication with all members of the treatment team to ensure continuity of care.
- Coordinate discharge planning, including preparing discharge summaries with patient instructions.
- Graduate of an accredited School of Nursing.
- Current active and unrestricted registered nurse license.
- At least 2 years of clinical experience working as a RN in an area requiring training and hands-on experience in infusion therapies.
- Current certification in Basic Life Support for Health Care Professionals.
- Bachelor of Science in Nursing from an accredited school of nursing.
- CRNI, IgCN, OCN highly preferred.
