Job Summary

We are seeking a skilled Discharge Planning Registered Nurse to join our team at Methodist Hospital Northeast. As a key member of our healthcare family, you will play a vital role in ensuring the smooth and timely discharge of patients.

Key Responsibilities
  • Facilitate patient throughput by identifying pending and confirmed discharges, utilizing tele tracking to enter identified discharges when units have not.
  • Communicate with the Patient Placement Center for direction on areas with priority for discharge, assisting those areas to expedite discharges.
  • Identify barriers related to timely discharge and communicate appropriately, assisting with discharge paperwork as needed to facilitate timely discharge of the patient.
  • Review with Charge Nurses anticipated discharges for the following day to assist with downgrading the patient to a lower level of care if appropriate.
  • Communicate with physicians and Case Management for anticipated discharge needs prior to the day of discharge.
  • Assist in identifying core measure patients and assure criteria have been met and completed prior to discharge.
Requirements
  • Graduate of an accredited diploma, associates, or baccalaureate degree registered nursing program.
  • Prefer Minimum 5 years of Clinical Nurse Experience.
  • Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date).
  • Current American Heart Association BLS Certification.
  • Meditech knowledge helpful.
  • Excellent communication skills.
