Job Description

As a Patient Care Coordinator, you will be responsible for ensuring that all residents receive high-quality care in accordance with federal, state, and local standards. Your duties will include:

  • Providing direct nursing care to residents, including administering medications, taking vitals, and performing various medical procedures.
  • Supervising nursing assistants and ensuring they follow established protocols and guidelines.
  • Developing and implementing individualized care plans for each resident.
  • Collaborating with other healthcare professionals to ensure seamless care coordination.
  • Maintaining accurate and up-to-date records and reports.

Required Skills and Qualifications

To be successful in this role, you must have:

  • A current LPN license in good standing.
