Position Title: Registered Nurse Care Coordinator

As a Registered Nurse Care Coordinator, you will deliver expert nursing care to patients, families, and caregivers as directed by physicians and in accordance with state regulations and organizational policies.

Key Responsibilities:

  • Continuously monitor and assess patient conditions.
  • Implement appropriate preventative and therapeutic nursing interventions.
  • Document and maintain accurate clinical records regarding patient care and status.
  • Assist patients with daily living activities.
  • Collaborate with patients, families, caregivers, and team members to ensure seamless care continuity.

Work Schedule:

  • 4 (10-hour) or 5 (8-hour) days, Monday through Friday.
  • Participation in weekend and holiday rotations.
  • This role encompasses service in designated counties.

Qualifications:

  • Graduate from an accredited nursing program.
  • Active Registered Nurse license in the State of Michigan.
  • At least one year of nursing experience, with a focus on hospice, medical, surgical, or critical care.
  • Current Basic Life Support (BLS) certification.
  • Experience dealing with terminal illness, end-of-life issues, and pain management.
  • Valid Michigan driver’s license and access to a reliable, insured vehicle for work purposes.
  • Negative TB test and/or chest X-ray, along with other required health screenings.
  • Up-to-date immunizations as mandated by LARA and agency policies.
