Key Responsibilities

The Patient Care Coordinator acts as a patient advocate throughout the pre-admission phase. Under the guidance of the Director of Business Development, you will collaborate with clinical teams to assess both current and prospective patients' conditions and healthcare requirements. You will serve as a vital resource for healthcare professionals and cultivate productive relationships within medical facilities.

Our facilities provide a comprehensive post-ICU care plan that emphasizes mobility, nutrition, and overall wellness - essential elements in helping patients regain their health. Patient Care Coordinators assist local hospitals in identifying patients who have entered a state of chronic critical illness and would benefit from this specialized level of care.

  • Utilizing a robust clinical background, knowledge of insurance processes, and problem-solving abilities, you will collaborate with physicians, discharge planners, and families to devise the most effective continuum of care.
  • Conduct clinical evaluations of referred patients and provide guidance to families to help them make informed care decisions.
  • Work alongside hospital case managers to facilitate seamless patient transfers and address any insurance-related or operational challenges that may hinder appropriate admissions to this specialized care.

This is a salaried role with a standard workweek from Monday to Friday, including occasional on-call duties.

Qualifications

We are looking for motivated team players who are dedicated to delivering high-quality care in all aspects of their work.

Minimum qualifications include:

  • Current Registered Nurse (RN) or Respiratory Therapist (RT) License.
  • Other educational and clinical credentials as approved by the Vice President of Business & Market Development or their designee.

Preferred qualifications that will enhance your success:

  • Possess investigative and analytical skills to navigate challenges that may impede patient admissions to optimal care levels.
  • Exhibit flexibility, self-motivation, accountability for outcomes, independence, and support for team initiatives.
  • Prior experience in sales or business development is advantageous.
