About the Role

We are seeking a skilled Physical Therapist to join our Home Care Team. As a Physical Therapist with our team, you will provide comprehensive and compassionate skilled therapy assessments to assigned clients.

Responsibilities
  • Patient Evaluation: Performs skilled therapy assessments and prescribed treatments to assigned clients based on MD orders.
  • Treatment & Planning: Orders, obtains prescribed equipment as needed, and teaches clients regarding the equipment obtained.
  • Communicates plan of care to the Primary Care Physician (PCP), and updates the PCP with changes in condition and subsequent alterations to the plan of care.
Requirements
  • Current New York State license or a limited permit as a Registered Physical Therapist.
  • Current and valid NYS Drivers license.
  • Master's Degree in Physical Therapy or Bachelor's degree with prior experience as a licensed Physical Therapist required.
  • Prior Physical Therapist experience.
Work Environment

This role is a Full Time position with a weekend enhanced rate. The schedule is 8AM-6PM (Saturday & Sunday); third day is negotiable. The location is Monroe County.
