Responsibilities:

  • Primary Care Delivery: Deliver high-quality, patient-centered care to assigned members, focusing on cost-effectiveness and coordination with other healthcare professionals.
  • Care Coordination: Ensure seamless transitions between care settings, facilitating communication among providers, members, and caregivers.
  • Program Enhancement: Foster strong relationships with internal and external partners, promoting the CCM program and driving membership growth.
Requirements:
  • Certified Nurse Practitioner or Physician Assistant through a national board.
  • Active and unrestricted license in the state where you reside.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
  • Access to reliable transportation for travel between care sites.
  • Ability to lift 30 pounds and navigate various dwelling conditions.
