About the Role

Tribute Home Care is seeking a dedicated and compassionate Care Manager to join our Massachusetts team. As a key member of our team, you will play a vital role in providing exceptional care and support to our elderly clients.

Key Responsibilities
  • Provide warm and helpful introductions to Tribute through prompt responses to inquiries and in-person assessments.
  • Oversee the setup of Tribute's services by formulating client care plans, matching caregivers with clients, and orienting caregivers to client care plans.
  • Ensure regular, proactive communication with all parties involved in each client's care.
  • Revising care plans as necessary to ensure the best possible outcomes for our clients.
  • Develop strong and trusting relationships with Tribute's clients and their families.
  • Build relationships with community referral sources to promote and grow our services.
Requirements
  • Bachelor's degree from an accredited college.
  • Experience working in a clinical setting.
  • Prior experience in caseload management.
  • An eagerness to impact growth and make a difference in the lives of our clients.
  • Unwavering commitment to customer experience and service excellence.
  • Strong writing and communication skills.
  • Strong attention to detail and organization.
  • Collaborative spirit and ability to work effectively in a team environment.
