MedStar Medical Group is seeking a highly skilled Population Health Manager to lead the day-to-day coordination, administration, and community engagement for Community Health Initiatives. The successful candidate will report to the RN Regional Director Population Health and be responsible for managing the implementation of Population and Community Health initiatives.
Key Responsibilities- Departmental Goals and Objectives: Contribute to the achievement of established department goals and objectives, adhering to department policies, procedures, quality standards, and safety standards.
- Population Health Initiatives: Act as the primary agent for the coordination and implementation of Population and Community Health initiatives, including developing policies, procedures, goals, and objectives.
- Community Health Needs Assessment: Support community health needs assessment and community benefit processes.
- Health Education Programs: Assess needs, analyze, and coordinate health teaching and education programs for target populations.
- Mentorship: Mentor nursing and allied health students.
- Data Analysis: Assist in the synthesis and presentation of data into clear, relevant, and visually appealing materials for strategic decision-making.
- Grant Applications: Participate in grant application processes for the sustainability of community health initiatives.
- Advocacy: Support advocacy and other activities with partners to meet strategic goals.
- Outreach and Research: Coordinate outreach, research, and other activities with initiative partners to minimize redundancies and align strategies and actions.
- Reporting and Record-Keeping: Create and manage summary reports of community health programs, ensuring compliance with regulatory agencies' requirements.
- Representation: Participate in meetings, committees, local coalitions, steering groups, and advocacy groups, representing the department and hospital in community outreach efforts.
- Quality Improvement: Participate in multi-disciplinary quality and service improvement teams.
- Resource Provision: Serve as a resource to colleagues and consumers of healthcare.
- Supervision: Supervise and provide vaccinations and health screenings, including vital signs, body mass index, visual/hearing acuity, and carbon monoxide levels.
- Program Management: Provide program management assistance at regional facilities as assigned.
- Education: BSN required from an accredited program; Master's degree in a health-related field from a nationally accredited school/program preferred.
- Experience: 3-4 years of work experience, including project management, population health coordination, and staff development or continuing education.
- Licenses and Certifications: RN - Registered Nurse - State Licensure and/or Compact State Licensure; CHES - Certified Health Education Specialist; MCHES - Master Certified Health Education Specialist.
