Job Title: Perinatal Services Case Manager

Community of Hope is seeking a highly motivated and compassionate Perinatal Services Case Manager to join our team. As a key member of our Maternal and Child Health department, you will play a vital role in ensuring access to supportive and educational services for prenatal and postnatal patients.

Job Summary:

The Perinatal Services Case Manager will work closely with our perinatal navigators to provide care coordination services to ensure access to perinatal health and early child health services. This includes conducting thorough assessments, providing health education, and coordinating social services and parenting resources.

Key Responsibilities:
  • Maintain a case load of approximately 30 participants and provide care coordination services to ensure access to perinatal health and early child health services.
  • Conduct thorough assessments on all prenatal/postpartum clients using OB Authorization forms, Healthy Screening tools, and other relevant assessments.
  • Provide health education to clients in various settings, emphasizing healthy pregnancy, self-management of care, and the healthcare system.
  • Utilize non-COH data sources, including CRISP, to coordinate care for high-risk clients and ensure providers have access to accurate and complete medical information at the point of care.
  • Assist clients in setting short- and long-term goals using motivational interviewing strategies.
  • Develop care plans with designated periods, emphasizing risk factors such as social needs, barriers to care, pregnancy planning, and food insecurity.
  • Be available for home and/or hospital visits as determined by the Homeless System Perinatal Services program.
  • Ensure all prenatal and postpartum clients utilize referrals provided through weekly reminder calls and assist with provider appointments, including rescheduling missed appointments and ensuring access to care.
  • Act as a liaison between clients, families, homeless services providers, clinical staff, and other healthcare professionals.
  • Regularly facilitate individual and group centering and on-site health education with clients as needed.
  • Work with Enrollment Specialists to ensure clients are enrolled in medical insurance and connected to their Managed Care Organizations case management services department.
  • Promote breastfeeding during interactions with clients and provide breastfeeding support, including assessing latching and feeding processes, providing corrective interventions, and counseling mothers.
  • Participate in quality management and evaluation activities as requested.
  • Manage client information in compliance with HIPAA, HSRA, and other privacy regulations, ensuring appropriate releases are completed to facilitate information sharing.
  • Follow up with clients or healthcare providers (with client permission) to ensure successful referral and attendance at appointments.
  • Attend Maternal and Child Health department meetings at Community of Hope and all-staff meetings.
  • Participate in Virginia Williams Resource Center staff huddles and HONEY team meetings.
  • Document any relevant client encounters, including face-to-face, telephone, and electronic communications, in eCW or Quickbase within 3 business days and other systems per HIPAA and departmental policies and procedures.
  • Respond to all urgent telephone encounters and messages within 1 business day and 3 business days for all others.
  • Secure client information and maintain client confidence by completing and safeguarding medical records and keeping client information confidential.
  • Complete other duties as assigned by supervisor.
Requirements:
  • B.A./B.S. or equivalent experience required; B.A./B.S. in social work, education, or related field preferred.
  • Experience providing social services, care coordination, or healthcare education required.
  • Familiarity with perinatal services and standard of care required.
  • Strong organizational skills, ability to multi-task, and problem-solving skills.
  • Strong written and verbal communication skills.
  • Experience working with under-resourced populations, including pregnant people, preferred.
  • Familiarity with Medical Assistance system and community resources strongly preferred.
  • Ability to travel between sites required.
  • Willingness and ability to make home visits and hospital visits, work occasional evenings and weekends.
  • Proof of required vaccinations is required.
