Responsibilities:

Conduct brief cognitive and emotional assessments to identify patient needs
Collaborate with nursing home staff to develop and implement individualized care plans
Provide psychotropic medication management and gradual dosage reductions as needed
Work closely with our team of Board Certified Geriatric Psychiatrists, Psychiatric Nurse Practitioners, Licensed Psychologists, and Licensed Clinical Social Workers to ensure comprehensive patient care
