Job Overview

The Intensive Case Management Specialist for Children's Services is responsible for conducting and coordinating assessments, referrals, advocacy, and support for patients and their families. This role ensures that clients receive high-quality, intensive services tailored to their healthcare needs, free from obstacles. The specialist will embody and promote the values and mission of ICAN Inc. through the execution of various duties and responsibilities.

Key Responsibilities:

  • Deliver Intensive Case Management services to individuals facing significant challenges, particularly those transitioning from hospital care or other institutional environments.
  • Collaborate with local social services to perform assessments, identify needs, and establish ongoing referrals for clients.
  • Provide a supportive framework for individuals awaiting discharge, ensuring their safety and facilitating connections to community services to avert future hospitalizations.
  • Assist clients in planning for a secure and effective reintegration into the community post-discharge.
  • Engage in multidisciplinary case conferences with hospital staff and social services to address immediate safety concerns and assist with accessing necessary benefits and housing support.
  • Create a Safety Plan and Care Plan based on initial assessments to identify essential services and linkages.
  • Maintain a caseload for up to 90 days, monitoring clients through virtual and in-person visits to ensure effective implementation of their discharge plans.
  • Establish and maintain all initial service linkages, advocating for additional resources as needed.
  • Work collaboratively with service providers to develop a communication strategy among the care team.
  • Continuously monitor client goals and ensure effective communication within the team.
  • Ensure that care plans align with Intensive Case Management standards.
  • Consult with family members and support networks to maintain consistent support for clients.
  • Keep accurate case documentation and client information within the designated electronic health record system.
  • Assist clients in transitioning to programs that meet their needs and enhance their safety and well-being.
  • Additional responsibilities may be assigned as necessary.

Qualifications:

  • A Bachelor's degree in Human Services, Mental Health, or a related field is preferred.
  • A minimum of two years of experience in Human Services, focusing on Mental Health and Substance Abuse.
  • Proficiency in Microsoft Office Suite, Google Suite, and relevant case management software.
  • A valid driver's license is required.
