Job Description

Job Summary:

The Care Partner will provide personalized care and support to residents, upholding the philosophy of assisted living and promoting their overall well-being. This role involves assisting residents with their activities of daily living in a professional and caring manner, adhering to federal, state, and local regulations, as well as Oaks Senior Living policies and procedures.

Key Responsibilities:

Resident Services:

1. Provide high-quality care and assistance to residents, aligning with the Oaks Senior Living philosophy of Person-Centered Lifestyle and respecting resident rights.

2. Assist residents with daily living activities, taking into account their individual preferences and needs.

3. Maintain confidentiality of personal and medical information of residents and colleagues.

4. Adhere to each resident's Negotiated Service Plan, notifying the Shift Supervisor, Wellness Director, and/or Executive Director of any changes in the resident's condition.

5. Respond promptly to resident calls and requests.

6. Encourage and support residents in participating in activities, promoting their engagement and socialization.

7. Assist with meal service, ensuring residents' dietary needs are met, and observing and reporting changes in their appetite and physical abilities to eat and drink.

8. Maintain clean and sanitary conditions in resident, employee, and public bathrooms as needed.

9. Establish and maintain positive relationships with residents and their families.

10. Dispose of incontinent products according to policy.

11. Practice proper body mechanics when assisting with resident transfers and follow safety guidelines according to Oaks Senior Living policies and procedures.

12. Respond to resident emergencies, following proper policy and procedure.

13. Offer choice, maintain dignity, and promote individuality for all residents.

14. Protect and maintain resident rights.

15. Obtain 24 hours of continuing education in the initial year of employment and 16 hours annually, thereafter. Maintain CPR & First Aid certification.

16. Practice infection control policies and established safety precautions.

17. Participate in special events, activities, and marketing programs.

18. Carry out other duties as assigned by the Shift Supervisor, Wellness Director, MOD, or Executive Director.

Communications:

1. Attend all regular staff meetings and required in-service training sessions.

2. Communicate effectively to convey information to residents, Shift Supervisor, and Management.

3. Review daily any and all communication tools used in providing resident care.

4. Communicate residents' needs with Shift Supervisor and carepartner staff at change of shift.

5. Notify your supervisor or the Wellness Director if you observe a change in a resident's condition.

6. Report all supply needs.

7. Report hazardous conditions and equipment to the Executive Director.

8. Respond to on-the-job injuries in accordance with the community's policies and procedures.

9. Notify your supervisor of any concerns or questions presented by the residents' family members.

10. Maintain a positive attitude.

Reports to: Shift Supervisor, Wellness Director, Manager-on-Duty, and Executive Director

Qualifications:

1. Experience in caring for older adults is preferred.

2. Ability to read, write, and speak English.

3. Desire to work with older adults, possessing a friendly and caring disposition.

4. Must be 18 years of age. Must have a satisfactory criminal history check.

5. Must have a physical exam by a licensed physician. Must have a negative drug screen.

6. Must understand the risk of exposure to infectious waste, disease, chemicals, solutions, and disinfectants.

7. Must be able to react in an emergency situation.

Physical Job Requirements:

To fulfill job requirements, the employee must be able to perform key physical tasks for extended periods. Employees must adhere to proper body mechanics and safety measures when supporting or transferring residents. In a typical eight-hour day, the employee will:

1. Stand/walk up to eight hours a day.

2. Sit up to two hours a day.

3. Frequently support up to 70 pounds.

4. Occasionally lift/carry up to 50 pounds.

5. Frequently kneel, bend, and reach.

6. Secure proper assistance for transferring of residents as needed.
