Position Overview:
We are seeking a dedicated Full-Time Certified Occupational Therapy Assistant to provide exceptional care in the home health sector.

About Us:
At Phoenix Home Care and Hospice, our therapy team is committed to enhancing the daily lives of our clients through personalized and compassionate care.

Work Schedule:
Full-Time; Day shifts, Monday through Friday.

Compensation:
Annual salary ranging from $65,000 to $80,000.

Key Responsibilities:
  • Deliver occupational therapy services to patients under the supervision of a Licensed Occupational Therapist, adhering to the physician's Plan of Care and relevant regulations.
  • Engage in direct therapy services as prescribed, ensuring compliance with established standards.
  • Participate in communication and case conferences to enhance the quality and continuity of patient care.
  • Maintain adherence to the Business Code of Ethics and agency standards.
  • Document all care and services provided accurately and timely, in line with physician orders and agency protocols.
  • Collaborate with team members to review and update patient care plans, setting new goals as needed.
  • Perform any additional duties as assigned by the Supervisor.

Qualifications:
  • COTA certification is required.
  • Experience in home care or rehabilitation is preferred.
