About the Role:
The Registered Nurse (RN) is a key member of our team at Pathwell Health, responsible for delivering high-quality patient care in the comfort of their own homes. As a skilled and compassionate RN, you will assess patient health problems and needs, develop and implement individualized care plans, and maintain accurate medical records.
Key Responsibilities:
- Provide skilled nursing treatments as ordered by the client's physician, ensuring timely and effective care.
- Conduct thorough evaluations, including vital signs and medication reviews, to identify patient needs and develop targeted care plans.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
- Document patient observations, medical information, and vital signs accurately and thoroughly.
- Work closely with patients and their families to educate them on proper home health care procedures and strategies.
- Monitor patient symptoms and conditions, reporting any changes or concerns to the healthcare team.
- Administer medications and treatments as prescribed, ensuring patient safety and comfort.
- Prepare patients for and assist with examinations or treatments, promoting a positive and supportive care environment.
- Modify patient treatment plans as needed, based on patient responses and conditions.
- Order and interpret diagnostic tests to identify and assess patient conditions, making informed decisions about patient care.
- Monitor patient diet and physical activity, providing guidance and support to promote overall health and well-being.
- Engage in ongoing research and education to stay current with best practices in nursing and patient care.
Requirements:
- Associate's degree in Nursing required; Bachelor's degree preferred.
- Current and valid California Registered Nurse License.
- Prior nursing experience, particularly in home health, is preferred.
- Current CPR certification is required.
- Strong interpersonal skills, with the ability to build rapport and establish trust with patients.
- Computer literacy and strong organizational skills are essential.
- Excellent written and communication skills are required.
- Ability to meet the physical demands of the job, including lifting up to 50 pounds.
- Bilingual (Spanish) is preferred.
