Clinical Practitioner

We are currently hiring for a Clinical Practitioner role at Community Care Physicians. As a member of our medical team, you will be working closely with experienced physicians and other healthcare professionals to deliver high-quality patient care.
Job SummaryThis is a full-time position offering an attractive salary range of $120,000 - $180,000 per annum. If you're passionate about delivering excellent patient outcomes and have the necessary qualifications and experience, we encourage you to apply.
Responsibilities- Conduct thorough patient assessments and develop effective treatment plans.
- Provide expert care and support to patients with various medical conditions.
- Work collaboratively with multidisciplinary teams to ensure seamless patient care.
- Stay up-to-date with the latest medical knowledge and best practices through ongoing education and training.
- Licensure and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
- For Physician Assistants:
- Bachelor's degree in physician assistant studies and completion of a physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant.
- Passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
- Current certification by National Commission on Certification of Physician Assistants.
- At least one year of experience as a physician assistant in ambulatory healthcare.
- Current CPR certification.
- For Nurse Practitioners:
- Successful completion of a nurse practitioner education program acceptable to NYSED.
- Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
- Successful completion of pharmacology coursework acceptable to NYSED.
- Three years of professional nursing experience in an ambulatory setting preferred.
- At least one year of experience as a nurse practitioner in ambulatory healthcare.
- Current CPR certification.
