Job Title: Registered Nurse Case Manager

At Interim HealthCare of New Hampshire, we are seeking a skilled Registered Nurse Case Manager to join our team.

Job Summary:

The successful candidate will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.

Key Responsibilities:
  • Manage a caseload of patients and provide direct care and support
  • Conduct thorough assessments and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families
  • Document patient progress and outcomes
Requirements:
  • Valid nursing license in the state of New Hampshire
  • 2+ years of experience in skilled care
  • 1+ year of experience in nursing supervision
  • Recent wound care experience
  • Ability to lift 50 pounds with no restrictions
  • Current CPR certification
  • Reliable transportation
