Job Title: Registered Nurse Case Manager

We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire Careers.

Job Summary:

The Registered Nurse Case Manager position is responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.

Responsibilities:
  • Manage a caseload of patients, providing direct care and comprehensive nursing services
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Conduct regular assessments and evaluations to monitor patient progress
  • Provide education and support to patients and their families
Requirements:
  • Valid nursing license in the state of NH
  • 2+ years of experience in skilled care preferred
  • 1+ year of experience in nursing supervision preferred
  • Recent wound care experience
  • Ability to lift 50 LBS with no restrictions
  • Current CPR certification
  • Reliable transportation
