RN Case Manager

The RN Case Manager is a key member of our healthcare team, responsible for coordinating patient care and ensuring optimal health outcomes. This role requires strong clinical expertise, effective communication skills, and a commitment to delivering high-quality patient care.
Responsibilities- Conduct thorough medical necessity screenings and submit for Physician Advisor review
- Coordinate patient care across the continuum, ensuring seamless transitions and timely throughput
- Develop and implement transition plans, addressing patient needs and promoting safe discharge
- Lead and facilitate multi-disciplinary patient care conferences, fostering collaboration and communication among healthcare teams
- Manage concurrent disputes and make referrals to other departments as needed
- Identify and refer complex patients to Social Work Services, ensuring comprehensive support
- Communicate effectively with patients, families, and caregivers, providing clear and concise information about the plan of care
- Collaborate with physicians, office staff, and ancillary departments to ensure comprehensive patient care
- Participate in Complex Case Review, analyzing patient data and identifying opportunities for improvement
- Ensure timely and accurate documentation in the Case Management system, maintaining accurate patient demographic and insurance information
- Identify and document potentially avoidable days, promoting efficient resource utilization
- Report over and underutilization, contributing to data-driven decision-making
- Graduate of an accredited school of nursing
- 2 years of acute hospital patient care experience
- Preferred: Acute hospital case management experience and accredited Case Manager (ACM) certification
This role requires frequent sitting, moderate standing, walking, reaching, stooping, and bending. Manual dexterity, mobility, touch, and auditory skills are necessary to perform all related duties.
