Job Description

The Housing Case Manager plays a vital role at Elevate Community Services Inc, providing expert case management and navigation services to individuals residing in our emergency shelter, Travel Inn.

This position involves working closely with clients to identify and address barriers to housing, such as poor credit, criminal justice involvement, and pending evictions. The Housing Case Manager will guide and support clients through the housing search process, from document collection to securing a lease.

Key Responsibilities:

  • Coordinate services for clients to ensure seamless support throughout the housing process
  • Understand and support clients through the community's Coordinated Entry System (CES)
  • Identify and address barriers to housing, including poor credit, criminal justice involvement, and pending evictions
  • Guide clients through the housing search process, including document collection, completing assessments, and signing a lease
  • Confirm client eligibility for housing programs and services
  • Locate housing opportunities in the community and encourage clients to proactively search for housing
  • Maintain accurate client files and complete documentation within the required timeframe
  • Collect program data, including tracking housing placements and case notes
  • Develop a working knowledge of available community resources and act as a liaison
  • Maintain relationships with referring and supporting agencies and represent the agency at community meetings
  • Advocate for the client's voice and choice in the housing process

Requirements:

  • Bachelor's Degree in social services or a closely related field
  • 3 years' experience in housing-related social services
  • Excellent documentation and communication skills
  • Ability to work well in a team environment
  • Adherence to the highest standard of ethical conduct, especially confidentiality
  • Culturally and linguistically sensitive to populations served
  • Clean driving record and valid driver's license
