Pharmacist - Long-Term Care Specialist

**Job Summary:**
We are seeking a skilled Pharmacist to join our team in Smyrna, GA. As a Closed-Door Pharmacy Professional, you will be responsible for evaluating physician medication orders and making recommendations for therapy changes.
About the Role:
- Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
- Maintain patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensure that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with healthcare team members.
- Collaborate with healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
Requirements:
- Bachelor's Degree in Pharmacy or Pharm D preferred.
- 1+ year experience in long-term care.
- Valid/Active Pharmacist license in Georgia.
- Knowledge of pharmacy laws, federal and state regulations concerning facilities and residents.
