Job Title: Home Care Occupational Therapist

Extended Home Care of NY is seeking an experienced and reliable Occupational Therapist to conduct home visits in Westchester County.

Job Summary:

The Occupational Therapist will provide therapy services to patients according to a written physician's plan of care. This may include assessing and evaluating therapeutic/rehabilitative/functional status, and participating in the development of the total plan of care.

Responsibilities:
  • Assess occupational therapy needs by applying objective diagnostic and prognostic tests.
  • Prepare initial assessments of patients' conditions and perform periodic re-assessments.
  • Develop plans of care in accordance with authorized practitioners' prescriptions for therapy services.
  • Provide occupational therapy treatment according to established goals, in collaboration with patients' physicians.
  • Document patients' responses to treatment and modify treatment plans and objectives as needed after notifying patients' physicians.
  • Instruct healthcare personnel, patients, and families in techniques and principles necessary for safe management and maximum restoration of mobility and function.
  • Complete case conferences, 60-day, and transfer and discharge summaries when indicated.
  • Prepare all documentation on appropriate forms.
  • Participate in the development and periodic revision of physicians' plans of treatment with Home Care Coordinators.
  • Participate in case conferences, staff meetings, and in-service programs as needed and as designated by the Rehab Manager.
  • Work with other members of the Interdisciplinary Team to develop appropriate interventions to achieve clinical and functional goals.
  • Participate in the agency's Quality Assurance Performance Improvement/PIP as assigned.
  • Submit visit notes, at least weekly, or within 7 days after completion of each home visit.
  • Provide written Home Exercise Programs to clients and Home Care Coordinators and instruct patients/families/caregivers as per physicians' instructions.
  • Demonstrate sound judgment by taking appropriate actions regarding suspected violations of corporate compliance regulations.
  • Report all suspected violations to supervisors, Compliance Officers, or Compliance Hotlines.
Requirements:
  • Graduate of an Occupational Therapy school approved by the American Occupational Therapy Association and the Council on Medical Education.
  • License and current registration to practice as a Registered Occupational Therapist in New York State.
  • Two years of institutional and/or home care experience required.
  • Satisfy state health status requirements.
