Comprehensive Care is seeking a highly skilled and compassionate Care Coordinator - Scheduler to join our team. As a key member of our care coordination team, you will play a vital role in ensuring that our clients receive exceptional care and support.
Key Responsibilities:
- Schedule and coordinate daily activities of caregivers and clients to ensure seamless care delivery.
- Build strong relationships with field staff to understand their preferences and abilities, enabling effective scheduling and care planning.
- Review open shifts daily, prioritizing and scheduling all open shifts to meet client needs.
- Provide caregivers and clients with accurate and timely information regarding their schedules and any changes.
- Collaborate with the Executive Director and Care Coordinator Manager to ensure that care and staffing needs are met, and that our clients receive the highest level of care.
Requirements:
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, caregivers, and internal stakeholders.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Ability to work in a fast-paced environment, with a high level of adaptability and flexibility.
- Knowledge of care coordination principles and practices, with a focus on delivering exceptional client care.
