We are seeking a skilled and compassionate Registered Nurse to join our team at CenterWell Home Health. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in the comfort of their own homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients with chronic conditions or recovering from illness, injury, or surgery
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
  • Conduct regular assessments, evaluations, and updates to ensure optimal patient outcomes
  • Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to coordinate patient care
  • Document patient progress, outcomes, and any changes to care plans
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or a related field
  • Strong med-surg, ICU, ER, or acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Valid driver's license, auto insurance, and reliable transportation
