Key Responsibilities- Provide comprehensive nursing care to patients with chronic conditions or recovering from illness, injury, or surgery
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Conduct regular assessments, evaluations, and updates to ensure optimal patient outcomes
- Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to coordinate patient care
- Document patient progress, outcomes, and any changes to care plans
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum one year of nursing experience, preferably in home health or a related field
- Strong med-surg, ICU, ER, or acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
