Job Title: Home Care Occupational Therapist

We are seeking an experienced Home Care Occupational Therapist to join our team at Liberty Home Care. As an Occupational Therapist, you will play a vital role in helping our patients recover and regain their independence in the comfort of their own homes.

Job Summary:

The Home Care Occupational Therapist will provide patient care services, including direct treatment and follow-up, to patients in their homes. You will evaluate patients new to service, develop a plan of care, and establish measurable goals in compliance with patient's physical assessment. You will also document patient care data accurately and thoroughly, and communicate with intercompany and intracompany contacts as necessary.

Responsibilities:
  • Evaluate patients new to service and develop a plan of care
  • Provide direct treatment and follow-up to patients in their homes
  • Establish measurable goals in compliance with patient's physical assessment
  • Document patient care data accurately and thoroughly
  • Communicate with intercompany and intracompany contacts as necessary
  • Coordinate with Patient Care Coordinator in making referrals to appropriate disciplines
  • Provide in-service education, health care professional education, and public education as needed
Requirements:
  • Minimum bachelor's degree in occupational therapy from an accredited school of occupational therapy
  • Successful completion of the National Certification Exam and current licensure from the state where therapy will be provided
  • Prefer a minimum of one year's therapeutic experience in a non-home health care environment
  • Strong knowledge of theory and concepts relating to the practice of physical therapy
  • CPR certified and strong communication and organization skills
  • Minimum of twelve hours of continuing education annually
