Job Title: Home Care Occupational Therapist

At Liberty Home Care, we are seeking an experienced Home Care Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in helping our patients recover and regain their independence in the comfort of their own homes.

Job Summary:

We are looking for a highly skilled and compassionate Occupational Therapist to provide patient care services, including direct treatment and follow-up. The ideal candidate will have a strong knowledge of occupational therapy principles and practices, as well as excellent communication and organizational skills.

Responsibilities:
  • Evaluate patients new to service and develop a plan of care appropriate to their diagnosis.
  • Provide patient care services, including direct treatment and follow-up.
  • Coordinate with other healthcare professionals to ensure comprehensive care.
  • Document patient care data accurately and thoroughly.
  • Communicate effectively with patients, families, and healthcare teams.
Requirements:
  • Minimum bachelor's degree in occupational therapy from an accredited school.
  • Current licensure from the state where therapy will be provided.
  • Prefer a minimum of one year's therapeutic experience in a non-home health care environment.
  • CPR certification and strong communication and organization skills.
