Key Responsibilities- Conduct thorough assessments to identify members' needs and develop personalized plans to optimize their health outcomes.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and patient-centered approaches.
- Provide education and support to members on health plan benefits, community resources, and self-management strategies.
- Maintain accurate documentation of services and care plans in accordance with department policies.
- Participate in program development, orientation, and educational activities to enhance members' capacity for self-directed care.
- Master's degree in Social Work from an accredited university.
- Minimum one year of Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting.
- Successful completion of the Kaiser Permanente NW Social Worker Internship program.
- Licensure requirements: Masters Social Worker License (Oregon) within 6 months of hire, or Clinical Social Work Associate Certificate (Oregon) within 6 months of hire, or Licensed Clinical Social Worker (Oregon) within 6 months of hire.
