Job Summary

CenterWell Home Health is seeking a skilled Medical Social Worker to join our team. As a Medical Social Worker, you will play a vital role in evaluating and developing personalized plans of care for our patients, addressing their emotional and social needs. Your expertise will help patients and families navigate the healthcare system, access community resources, and achieve optimal health outcomes.

Key Responsibilities
  • Evaluate patients' social and emotional states, identifying areas for improvement and developing strategies for care.
  • Assess patients' relationships with healthcare providers, financial resources, and available community resources.
  • Provide education and support to patients and families, empowering them to manage their health and well-being.
  • Collaborate with interdisciplinary care teams to develop and implement comprehensive care plans.
  • Supervise and mentor Social Work Assistants, ensuring high-quality care and adherence to company standards.
Requirements
  • Masters or doctoral degree in social work from a Council on Social Work Education-accredited program.
  • Current licensure as a social worker in the state of practice.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • Minimum one year of experience as a social worker in a healthcare setting, home health, or hospice.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
