As a key member of our healthcare team, the Case Manager will be responsible for providing comprehensive care coordination services to patients and their families, ensuring seamless transitions throughout the healthcare continuum.
Key Responsibilities:
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' medical, social, and financial needs, identifying opportunities for improvement and developing strategies to address these needs.
- Facilitate communication between patients, families, and healthcare providers to ensure that all parties are informed and engaged in the care planning process.
- Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services and support.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.
- Participate in quality improvement initiatives to enhance the overall quality of care and patient satisfaction.
Requirements:
- Bachelor's degree in nursing or related field.
- Minimum of 5 years of experience in case management or a related field.
- Current RN licensure in the state of Texas or compact state licensure.
- ACM or CCM certification preferred.
