  • Conduct thorough assessments of patients' medical, social, and financial needs, identifying opportunities for improvement and developing strategies to address these needs.
  • Facilitate communication between patients, families, and healthcare providers to ensure that all parties are informed and engaged in the care planning process.
  • Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services and support.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance the overall quality of care and patient satisfaction.

Requirements:

  • Bachelor's degree in nursing or related field.
  • Minimum of 5 years of experience in case management or a related field.
  • Current RN licensure in the state of Texas or compact state licensure.
  • ACM or CCM certification preferred.
