Responsibilities:- Provide direct patient care, evaluating outcomes and adjusting nursing care processes as needed.
- Establish and implement individualized plans of care in collaboration with patients, families, and healthcare teams.
- Coordinate case management activities within interdisciplinary healthcare teams to ensure seamless delivery of services.
- Communicate effectively with patients, families, and healthcare teams to ensure optimal patient satisfaction and outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Current licensure as a Registered Professional Nurse in the state where the facility is located or in a state covered by a licensure compact agreement.
- Zero to two years of experience in Home Health or Hospice Care, with a preference for experience in Acute Care or Skilled Care Facilities.
- BSN preferred, but not required.
- Ability to establish and maintain positive relationships with patients, families, and healthcare teams.
- Strong clinical judgment, critical thinking, and problem-solving skills.
