Job Title: Foster Care Programs Case Manager I

At Elevance Health, we are seeking a highly skilled and compassionate Foster Care Programs Case Manager I to join our team. As a key member of our organization, you will play a vital role in providing exceptional care and support to our members.

Job Summary:

The Foster Care Programs Case Manager I is responsible for performing case management telephonically and/or by home visits within the scope of licensure for special programs, such as Foster Care. This role requires a strong understanding of healthcare systems, social determinants of health, and community resources.

Key Responsibilities:
  • Conduct assessments to identify individual needs and develop comprehensive care plans to address objectives and goals.
  • Support member access to appropriate quality and cost-effective care and modify plans as needed.
  • Coordinate with internal and external resources to meet identified needs of the member in terms of integrated (physical and behavioral) whole person care.
  • Evaluate health needs and identify applicable services and resources in conjunction with members and their families.
  • Provide important information, including patient education, medication reconciliation, and identification of community resources, and assist with arrangement of follow-up care.
Requirements:
  • Requires MS/MA in social work, counseling, or a related behavioral health field or a degree in nursing and minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment.
  • Current active unrestricted license as an RN, LCSW (as applicable by state law and scope of practice), LMHC, LPC (as allowed by applicable state laws), LMFT, or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.
Preferred Skills:
  • Experience working with specialty populations preferred.
  • Clinical experience in social work counseling with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred.
  • Travels to worksite and other locations as necessary is preferred.
Work Environment:

This position will work in a hybrid model (remote and office). The ideal candidate will live in the State of Kansas.
