Job Description

The LPN Team Case Coordinator will work under the direction of the team manager/designee to coordinate patient services. Responsibilities include prioritizing daily patient schedules, requesting additional services, and ensuring timely referrals.

Requirements
  • A valid NYS license as a Licensed Practical Nurse
  • One year of experience in home care or medical office settings
  • Proficient use of Microsoft Office
Work Environment

The LPN Team Case Coordinator will work in a fast-paced environment with frequent phone calls and interactions with field staff. Physical demands include medium work exerting 20-50 pounds occasionally and/or 10-25 pounds frequently.

Compensation

The estimated salary for this position is $23.85-$29.25 per hour, based on location and experience. Rochester Regional Health is an Equal Opportunity/Affirmative Action Employer.
