Job Summary

Nuvance Health is seeking an experienced Licensed Clinical Social Worker (LCSW) to fill a Clinical Care Coordinator position in Kingston, NY. The successful candidate will be responsible for coordinating patient care, assessing clinical readiness for transition, and providing ongoing assessment, monitoring, and evaluation to meet individual patient needs.

Key Responsibilities
  • Care Coordination/Facilitation and Transition Management: Coordinate multiple aspects of care to ensure patient overall health goals improve and psychosocial wellbeing.
  • Assess patient's clinical readiness for transition in collaboration with the healthcare team and informs the team of available resources based on patient's clinical needs.
  • Participate in the development and improvement of clinical initiatives focused on reducing hospital admissions or trips to the ED, or re-admissions and ensuring appropriate levels of care.
  • Maintain effective relationships and a positive outlook when interfacing with other departments, multidisciplinary healthcare team, patients/families, and community providers.
  • Act as an intermediary between patients, physicians, and community providing advocacy, accurate healthcare information, potential referral services, and proper treatment plans.
  • Provide clinical social work interventions, including counseling, bereavement, brief goal-focused therapy, and crisis intervention.
  • Assist the primary care team in developing care management processes, such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
  • Collaborate effectively with team members, develop good relationships both inside and outside of the team, and contribute ideas and support decisions made by the team and the organization.
Requirements
  • Master's Level Degree of Social Work
  • Minimum Experience: three to five years
  • Licensed Clinical Social Worker in NY and CT
