Job Opportunity

We are seeking a dedicated and compassionate Occupational Therapist to join our team at CareCo Shoreline. As a home care therapist, you will work independently with patients in their homes, providing one-on-one care and support.

Responsibilities
  • Perform home visits for patients in a home care setting
  • Develop and implement individualized plans of care for patients
  • Collaborate with multidisciplinary teams to coordinate patient care
  • Provide education and support to patients and their families
Qualifications
  • Current license as an Occupational Therapist in Connecticut
  • Valid driver's license and reliable transportation
  • Strong communication and interpersonal skills
  • Experience in home health care, including Medicare
