Job Summary:

We are seeking a skilled Occupational Therapist to join our team in New London County. As a home care specialist, you will work independently with patients in their homes, providing one-to-one therapy and support. This is a per diem opportunity with a flexible schedule, allowing you to choose the days and hours that suit you best.

Responsibilities:

Perform home visits for patients in a home care setting

Regularly re-evaluate the therapy needs of our clients, initiating, developing, implementing, and making necessary revisions to their plan of care

Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition

Teach, supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient's condition

Qualifications:

Current license as an Occupational Therapist in Connecticut

Valid driver's license

Reliable transportation and willingness to travel in New London County

Strong communication and interpersonal skills

Home health experience, including Medicare, preferred

Benefits:

Mileage paid between patient visits

Training, professional development, and technology

Benefits, including time off, health insurance, and an agency vehicle, dependent on the number of visits conducted each week
