RWJBarnabas Health is in search of a Subacute Care Practitioner to enhance our Skilled Nursing Facilities within the region.

Opportunities for Full-Time, Part-Time, or Per Diem roles are available.

This position offers a unique chance to work with a flexible Monday-to-Friday schedule, ensuring an excellent work-life balance while being part of New Jersey's largest integrated healthcare network.

The Subacute Care Practitioner will oversee post-acute and chronic health conditions, diagnose and monitor medical issues, and execute comprehensive patient care strategies. Collaboration with other advanced practice professionals, physicians, facility leaders, and support staff is essential.

Ideal candidates will possess:

  • A robust work ethic and a commitment to a collaborative, dynamic practice environment.
  • Engagement with specialists, case managers, social workers, and all parties involved in patient care and discharge planning.

Candidate Qualifications:

  • Certification as a Physician Assistant or
  • A Master’s degree in Nursing with NP Certification and a current NJ state nursing license; CDS and DEA credentials.
  • Preferred prior experience as an Advanced Practice Provider in a Hospitalist or Extensivist role within Skilled Nursing Facilities.
  • Experience in Palliative or Hospice care is advantageous.
  • Basic Life Support (BLS) and Advanced Life Support (ACLS) certifications; PALS if applicable.
  • Active membership in a professional organization is preferred.
  • Willingness to accommodate various scheduling needs, including extended hours, weekdays, weekends, and holidays.
  • No procedural tasks required.
