Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Patient Care Manager in Home Health. As a Patient Care Manager, you will be responsible for overseeing the delivery of high-quality patient care and ensuring that our patients receive the best possible outcomes.
Key Responsibilities- Oversee the delivery of patient care services in the home health setting
- Develop and implement individualized care plans that meet the unique needs of each patient
- Collaborate with healthcare teams to ensure seamless transitions of care
- Conduct regular patient assessments and evaluations to ensure optimal outcomes
- Provide education and support to patients and their families to promote self-care and independence
- Current licensure as a Registered Nurse (RN) in the practicing state
- Minimum of two years of experience in a home care setting, with management or supervisory experience preferred
- Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
