Responsibilities- Provide individualized patient care to patients with mental health, psychiatric, and addictive disease diagnoses.
- Assist patients through the continuum of care, utilizing knowledge of behavioral health issues.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Monitor patient progress and adjust care plans as needed.
- Document patient information and maintain accurate records.
- Bachelor's degree in psychology, sociology, social work, or related field.
- Previous experience providing care in an inpatient or outpatient psychiatric or addictive disease setting.
- Interest in psychiatric, mental health, and/or behavioral health.
- Passion for providing patient care in a team environment.
