Job Title: Case Manager Bi-Lingual

We are seeking a highly skilled and experienced Case Manager Bi-Lingual to join our team at Delta-T Group. As a Case Manager Bi-Lingual, you will be responsible for connecting with and working successfully alongside youth and families using your expert knowledge to help sustain program requirements within the client location.

Responsibilities:
  • Take a position of leadership in supporting program structure and all youth within the program
  • Demonstrate initiative in supporting and empowering the youth and families
  • Ensure client files, records, monthly reviews and individual Service Plans are accurate, updated and secure
  • Participate in live supervision with their clients and their families
  • Meet regularly with the Program Manager for Supervision
  • Demonstrate engagement and leadership in all weekly staff meetings
  • Assist clients with discharge and aftercare planning as they transition into their next step following the Program
  • Provide the Program Manager with timely and accurate documentation of all services provided including client progress reports and reports of problems that require immediate attention
  • Oversee and review client assessments: provide case management services: and oversee the development of individual treatment plans for clients and families
  • Other duties as assigned
Qualifications:
  • Bachelor's Degree required, Master's Degree preferred or currently pursuing advanced education in the fields of Social Work, Case Management, Counseling or another relevant Human Services field
  • 5 years experience in the human service field preferably working with adolescents and/or young adults
  • Expert knowledge of issues of and surrounding homelessness
  • Demonstrated ability to work with community leaders to raise funding of programs and skills to conceptualize and effectively lead complex projects
