Nurse - ICF Group Home Bronx Manhattan Queens

AHRC NYC is seeking a Registered Nurse RN to join our ICF Group Home team in Bronx, Manhattan, and Queens. As a Registered Nurse RN, you will play a vital role in evaluating and monitoring the health and safety needs of individuals with developmental disabilities, promoting their wellness, and ensuring compliance with federal and state regulations.
Responsibilities:- Evaluate and monitor health and safety needs of individuals with developmental disabilities.
- Coordinate medical care, monitor health needs, and arrange for follow-up care as needed.
- Assess health status, observe for signs and symptoms of discomfort, distress, or illnesses, and evaluate physical status changes.
- Communicate with physicians, families, interdisciplinary team members, and clinicians involved in providing services to individuals.
- Accompany individuals on medical appointments, advocate for and discuss health issues, and communicate with team members.
- Attend meetings with management, staff, families, and clinicians to discuss health-related issues.
- Develop individual-specific nursing plans and protocols, update and revise them as needed, and supervise AMAP administration of prescribed medication and routine treatments.
- Monitor medication administration records, treatment, and adaptive equipment records, and report significant issues to appropriate clinicians and team members.
- Monitor consumer dietary and nutritional needs, in-service and support staff in managing special meal and feeding, and observe meal times for appropriate food choices.
- Participate in re-admission intake reviews, discharge planning meetings, and write individual progress notes and annual nursing assessments.
- Ensure referrals are made for routine and special medical services, follow up on recommendations and orders, and monitor infection control and environmental changes.
- Current licensure as a Registered Professional Nurse (RN) in New York State.
- CPR certification required.
- Experience working with individuals with developmental disabilities preferred.
- Excellent written and oral communication skills.
- Ability to handle multiple priorities and work as a member of a team.
- A valid NY State or Tri-State Driver's License for more than one year, with a good driving record preferred.
