Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at DKMRBH Inc. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our staff to ensure the best possible outcomes for our residents.

Key Responsibilities
  • Administer medications and treatments in accordance with established protocols and procedures.
  • Provide safe and efficient personal care to residents, including bathing, dressing, and grooming.
  • Use recommended safety devices and follow infection control standards to prevent the spread of illness.
  • Respond to resident calls and requests for assistance in a timely and professional manner.
  • Document all care provided, including medications and treatments administered.
  • Participate in resident care planning and update care plans as needed.
  • Assess residents and document findings, reporting to the physician as necessary.
  • Transcribe physician orders and communicate resident changes to resident sponsors.
  • Maintain accurate records of nursing care and complete required documentation.
Requirements
  • Must be licensed as a Licensed Practical Nurse in Indiana and maintain licensure.
  • Must maintain first aid, mantoux, and CPR certification.
  • Ability to understand and communicate with residents and complete nursing forms.
  • Ability to follow verbal instructions and provide nursing care practices.
  • Ability to use medical equipment within scope of licensure.
  • Ability to provide safe and efficient resident care, following all applicable state and Veterans' Administration rules, IVH policies and procedures, and HIPAA rules.
Preferred Qualifications
  • Experience with Memory Care and short-term rehab.
  • Proficiency in computer skills and ability to learn other programs used in maintaining electronic medical records.
Working Conditions

Work is generally on a nursing care unit, in a controlled environment. May be exposed to body fluids and cleaning chemicals. May be exposed to violent residents.

Must be able to walk and stand for long periods of time. Must be able to bend, stoop, twist, reach above shoulders, push/pull, and assist with lifting and transferring residents, from 100 to 500+ pounds, using a gait belt or other accepted practices, or with a mechanical device.
