Job Summary

We are seeking a highly skilled and experienced Nurse Practitioner to join our team in Hospice and Palliative Care. The ideal candidate will have a strong background in palliative care and be able to work independently with RN support.

Key Responsibilities
  • Provide high-quality patient care in a fast-paced inpatient setting
  • Work collaboratively with the healthcare team to develop and implement individualized care plans
  • Conduct thorough assessments and develop treatment plans to meet the unique needs of each patient
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
Requirements
  • Certification in hospice and palliative care preferred
  • Self-motivated with excellent interpersonal skills
  • Ability to work independently with RN support
  • Monday-Friday schedule, inpatient position
