Breast Cancer Patient Navigator Job Summary

LMHS is seeking a highly skilled and compassionate Breast Cancer Patient Navigator to join our team. As a key member of our multidisciplinary team, you will serve as an advocate and educator to cancer patients from diagnosis through end of active treatment, and possibly end of life.

The ideal candidate will possess strong organizational skills, ability to prioritize and reprioritize quickly, and develop collaborative relationships both internally and externally. A strong leadership background, excellent interpersonal skills, and ability to work in teams and autonomously are essential for this role.

Key Responsibilities:
  • Facilitate appointments for patients with diagnostic mammograms, educating on additional testing or procedures necessary, including ultrasound or biopsy at the point of positive result for Breast Cancer.
  • Refer patients to Pulmonology for positive lung nodules.
  • Assist patients in understanding their diagnosis, treatment options, and resources available, including clinical research studies and facilitating surgical or medical oncology referrals as ordered by the physician.
  • Work with the Genetics Navigator to ensure patients are screened, educated, and genetic testing is obtained.
  • Post-Op, ensure patient follow-up has been established with medical oncology in a timely manner.
  • Serve as a link between patients and other care providers, coordinating multidisciplinary case conferences, and education of staff in Radiology and Oncology departments.

  • Registered Nurse license (RN)
  • Bachelor of Science in Nursing (BSN)
  • Maintain CPR certification
  • Oncology nursing experience preferred
  • Radiology experience beneficial
