Care Management Supervisor

Job Summary:

We are seeking a highly skilled Care Management Supervisor to join our team at Christiana Care. The successful candidate will provide direct care management services to a defined patient population, while providing supervisory services to the Case Management & Social Work staff.

Key Responsibilities:

  • Maintain assignment and support departmental activities for members of the Care Management team at any campus.
  • Identify and resolve team issues.
  • Work with the patient, family, and other members of the healthcare team to formulate a discharge plan that provides the patient with services in the appropriate post-acute care setting.
  • Gather and assess information regarding the patient's physical needs, mental status, family support system, financial resources, and available community and governmental resources.
  • Utilize assessment skills to develop a comprehensive care management plan that addresses the patient's identified needs.
  • Provide PTO, disability time, and FMLA oversight to ensure that staffing is optimal to meet service needs.
  • Provide direct supervision to department staff, including orientation, mentoring, coaching, and evaluation.
  • Evaluate staff development opportunities and competencies and perform management and evaluation tasks for all Care Management staff.
  • Coordinate and initiate training/orientation programs for staff and coordinate ongoing educational presentations in cooperation with other members of the department and the Staff Development and Education Committee.
  • Maintain employee files in cooperation with the department Manager/Director.
  • Communicate and interact with internal/external customers of the department, including Information Services, Financial Services, Ancillary departments, Third Party Payers, community social service agencies, healthcare facilities, CMS/QIO, and liaisons with the Department of Nursing.
  • Assist in the coordination and scheduling of meetings to support the needs of staff members and improve department service delivery.
  • Assist in the preparation, review, and monitoring of departmental budgets in cooperation/coordination with the departmental Manager and Director.
  • Participate in the development and maintenance of departmental policies and procedures.
  • Participate in Performance Improvement activities specific to departmental functions, system-wide initiatives, and regulatory guidelines.
  • Prioritize incoming requests/needs and forward to the Managers, Directors, or Vice President as needed.

Requirements:

  • Master in Social Work (or related field) or BSN in Nursing (Masters preferred)
  • Minimum 5-7 year experience in a healthcare environment, with at least 2 years serving in a leadership role
  • Licensed Social Work credential is preferred at hiring prior to June 11, 2024. Licensed Social Work credential is required in the State of Delaware effective June 11, 2024.
  • CCM/ACM Certification preferred. If certification not in place, willing to complete within one year of hire.
  • If RN- Licensed to practice in the State of Delaware required
