POSITION SUMMARY:


In alignment with the standards set forth by NCQA Medical Home Recognition and JACHO, SWLA has established a collaborative team of healthcare professionals dedicated to addressing the clinical needs of patients who select SWLA as their primary healthcare provider.

This team is composed of physicians, mid-level practitioners, and nursing support personnel as outlined in this job description.

To fulfill the responsibilities required of nursing support staff, SWLA will seek to recruit and employ qualified individuals in the roles of Medical Assistants, both Certified and Non-Certified, as well as Licensed Practical Nurses.

Compensation will be adjusted based on educational background, licensure, and relevant experience.

EDUCATION AND EXPERIENCE:
High School Diploma. Successful completion of a Practical Nursing program from an accredited technical institution.
All prerequisites set by the Louisiana Board of Practical Nursing must be fulfilled, if applicable. Current State Licensure, if required.
Current BLS/IV Certification. Continuing Education Units (CEUs) or equivalent contact hours for re-licensure or re-certification, if necessary. Must successfully pass a competency assessment.
Must exhibit a professional demeanor under stress.
Utilize correct grammatical English and write legibly to complete necessary documentation.

Present a professional appearance and maintain the highest standards of patient and community relations while representing the organization.


KEY RESPONSIBILITIES:
Actively participate as a vital member of the multidisciplinary care team.
Responsible for welcoming patients and ensuring minimal wait times in the examination area.

Gather essential information regarding the patient to assist the physician in delivering medical care.

This information includes the Chief Complaint and any specific physical or clinical issues that may require preliminary evaluation of the patient.

Ensure that patients are appropriately prepared for the type of visit they are attending.

Assist the physician with necessary procedures and examinations, administer medications within the scope of practice as directed by the physician, collect laboratory specimens, and perform assigned duties to provide comprehensive patient care.

Accountable for interpreting physician's instructions to the patient and/or family to address immediate health concerns.
Responsible for coordinating follow-up care as needed for patients' requirements.
Contact patients who have missed their appointments to reschedule.
Maintain the preparation and upkeep of examination rooms, clinic equipment, supplies, and the overall environment for effective operations.
Engage in training and health promotion initiatives as a member of the healthcare team.
Perform additional duties as assigned by the Director of Nursing, Chief Medical Officer, and designated Medical Provider.
Assume extra responsibilities as necessary to ensure the delivery of quality health services.

WORK SCHEDULE:
All medical staff adhere to our clinic operational hours.

Monday: 8:00 am to 5:00 pm
Tuesday: 8:00 am to 7:00 pm
Wednesday: 8:00 am to 5:00 pm
Thursday: 8:00 am to 5:00 pm
Friday: 8:00 am to 2:00 pm
Saturday: Closed
Sunday: Closed

COVID-19 POLICY:


SWLA Center for Health Services (SWLA CHS) has implemented a policy requiring all employees to be fully vaccinated against COVID-19 (including any booster shots if mandated), where permitted by law, unless an approved exemption is granted based on medical conditions or sincerely held religious beliefs that preclude vaccination.


If you are not fully vaccinated, you must have received at least one COVID-19 vaccination prior to your first day of employment and provide proof of your vaccination status on Orientation Day.

For the two-shot COVID-19 vaccination regimen, you will need to provide proof of your second vaccination within the first 45 days of employment.

In certain roles, you may be required to provide proof of full vaccination before commencing active work.

Failure to provide timely proof of your COVID-19 vaccination status may result in termination of employment with SWLA CHS.


If you are unable to be fully vaccinated due to medical conditions or sincerely held religious beliefs, you must apply for an exemption within the first 5 days of employment to maintain your position with SWLA CHS.

As part of this process, you will need to provide information or documentation regarding the reason for your inability to be vaccinated.

An approved exemption is required before you can begin active work.

If your exemption request is denied, you will be required to be fully vaccinated or your employment may be terminated.
